Pennsylvania FCCLA members must complete the "Website and Publication Waiver Form" (PA FCCLA 405) and submit to their adviser in order to be included in FCCLA publications. The adviser sends the Website and Publication Waiver Chapter Verification Form" (PA FCCLA 404) to the state office. These forms are required of all PA FCCLA members who attend Regional Leadership Meetings and the State Leadership Conference.
